Palliative and End of Life Care

Cheshire and Merseyside SCNs aim for those people who are approaching the end of their lives to be supported to live well before dying with peace and dignity in the place of their choice. The Palliative and End of Life Care Networks works to support this aim by: 

  • Raising awareness of and enabling conversations around death and dying with the public and professionals in health and social care 
  • Supporting and influencing the commissioning of high quality, cost effective pathways for end of life care 
  • Ensuring that health and care staff receive appropriate training so that they can support patients, their family members and carers 
  • Making sure that health and care services are joined up so that the patients' preferences can be supported.

 

Our Work Plans

The Network brings together specialists from a wide range of health and social care backgrounds to work together on a number of improvement programmes that we believe will make a difference to people who need palliative care or are approaching the end of their lives. These include: 

  • Working towards implementing Advance Care Planning (ACP) and Electronic Palliative Care Coordinating Systems (EPaCCS) across all services 
  • Providing additional support for health service providers to train more staff and improve standards. This also includes offering additional training for staff working across the region's care homes and in domiciliary care
  • Implementation of a unified 'Do Not Attempt Cardio-pulmonary Resuscitation' (uDNACPR) policy across the North West 
  • A regional campaign to raise awareness of the Network's Citizens' Charter which as been co-designed with local patient representatives
  • Supporting educational sessions for local hospices through the NHS Institute for Health's 'releasing time to care' programme
  • Working on a delivery plan for Multi-Professional Education and Training (MPET) which supports training in communication skills, care of the dying person, advance care planning and principles for end of life care.
  • Commission Marie Curie Palliative Care Institute to undertake Care of the Dying Evaluation (CODETM) on behalf of the Network to support carers and families
