North West End of Life Care Model

The model of delivery advocated by the North West Palliative and End of Life Care Operational Group uses a whole systems approach for all adults with a life limiting disease, regardless of age and setting, moving from recognition of need for end of life care, to care after death.

In order to apply the model, staff across organisations are required to undersand the needs and experiences of people and their carers.  In response to the publication of One Chance To Get It Right (2014) the North West Pallative and End of Life Care Operational Group have consulted with colleagues across the networks to revise the North West Model.

The North West Palliative and End of Life Care Operational Group has members from both the Cheshire and Merseyside Strategic Clinical Network and the Greater Manchester, Lancashire and South Cumbria Strategic Clinical Network.
