Induction Workshop

Time: Half day

Aim: To commence the Six Steps to Success Programme.

Objectives: By the end of the session the care home representative will be able to:

  • Identify the national, regional and local End of Life Care drivers
  • Understand the Six Steps to Success Programme
  • Commence the audit process
  • Have an understanding of their roles and responsibilities
  • Commence an End of Life Care Policy

End of Life Care Policy Template - policy template to assist the care home representative to develop the Care Home's End of Life Care Policy.

Knowledge, Skills and Confidence Pre-Programme Audit Form - pre-programme audit form to allow the care home representative to self-assess their levels of knowledge, skills and confidence at the beginning of the programme.

Quality Markers Pre-Programme Audit - allows the care home representative to audit the care home against Quality Markers and Measures detailed in the Quality Markers guidance (see audit pages).

Post Death Information Pre-Programme Audit Form allows the care home representative to capture information on deaths that have occurred in the last six months to demonstrate the effectiveness of the Programme.
