Advance Care Planning


Welcome to the web page for Advance Care Planning (ACP) for the Cheshire and Merseyside Palliative and End of Life Care Network.

This online facility has been designed to provide you with access to the ACP Framework and documents and some useful links to other resources.

If you are a member of the public you are welcome to explore the pages but be aware it is designed for professionals to use.  You may wish to dicuss the contents with one of your care team (e.g. General Practitioner or District Nurse).

What is Advance Care Planning?

Advance Care Planning is a process that gives people the chance to think about and write down what is important to them.

A person must have mental capacity* to make an advance care plan, and might find it helpful to talk to professionals about their care option (* for more information please visit and the National Archives)

As part of the process a person might choose to describe the type of care they would like at the end of their life.  For more information you may wish to look at the ACP Framework.


There are 3 forms linked to this framework:  

  • Record of Future Care Conversation - A form devised in Cheshire & Merseyside for recording conversations about future care only.  It can be used to record if a person does not wish to have conversations.
  • Advance Statement 'Planning for your future care'- Form developed locally to help people document their wishes or preferences about future care.
  • My Advance Decision to Refuse Treatment - Developed nationally. Written for patients to specify in advance which treatments they do not wish to have in future.

Advance Care Planning Sample Policies

There are several useful sample policies that we have agreement from those organisations providing to be share as an example.  In particluar, from within our region, the End of Life Partnership have provided a policy that can be adapted locally and this is available to download as a word document below.  You may also want to review the other sample policies, accessed via the links below, to ensure any policy you implement is robust.
